What You’ll Need for Your Visit
To help us process your insurance quickly and correctly, please bring the following to every appointment:
- Valid insurance card
- Photo ID for the parent or guardian
- Co-pay (if required by your plan)
- Completed new patient forms (if applicable)
Note: We also recommend calling your insurance provider in advance to understand your benefits, co-pays, and deductible details.
Billing & Payment Policy
To ensure smooth, timely service, please review our billing and payment policies below:
Bring Valid Insurance Info to Every Visit
Please bring your most current insurance card to every appointment. Outdated or incorrect information may delay claims processing—and you may be responsible for the full cost of the visit.
If your insurance plan requires specific forms, it is your responsibility to bring them with you. While we’re happy to help with documentation, we cannot guarantee payment or negotiate claims on your behalf.
Payment Is Due at Time of Service
Payment is collected at check-in and includes:
- Co-pay, co-insurance, or deductible (as specified by your insurance plan)
- Any outstanding balance on the account
We accept the following payment methods:
- Cash
- Check (Note: $25 fee for returned checks. No checks will be accepted after a returned check.)
- MasterCard, Visa, Discover
For minors: The adult accompanying the child is responsible for payment at the time of service, regardless of legal guardianship. If someone other than a parent or guardian brings your child, please ensure they are aware of this policy.
